Abstract

Introduction: Obsructive Urolithiasis cause hydronephrosis in the affected kidney and if left untreated, it disrupts renal function, leading to poor outcomes. The mini-Percutaneous Nephrolithotomy (PCNL) is considered a safer and effective procedure compared to conventional PCNL and Extracorporeal Shock Wave Lithotripsy (ESWL) because of its one-time procedure and lower failure rate.

Objective: To determine the effectiveness of the whirlpool mechanism in terms of stone clearance rate in mini-PCNL.

Methodology: This study was conducted in the Department of Urology, Institute of Kidney Diseases, Peshawar on 139 patients with 10–20 mm symptomatic renal stones planned for mini PCNL. Data on stone clearance rates, operative time, hospital stay, and complications was collected and analyzed using SPSS version 26, with a p-value ≤0.05 considered statistically significant.

Results: Mean age of the patients was 33.99±12.10 years. The mean stone size was 15.8±3.2 mm. Operative time was 56.01±8.72 minutes, and the mean hospital stay was 2.90 ± 1.20 days. Most of the patients were males (54.7%) and in the 15-30 years age group (43.8%), with a positive family history of stones (87.8%). About 56.1% had >15 mm stones, and 87.8% stones were cleared. The stone clearance had a significant relationship with the size of the stone (p=0.03), post-operative complications (p=0.001) and family history (p=0.05).

Conclusion: The whirlpool mechanism is a safe and effective modality for achieving stone clearance in patients undergoing mini-PCNL. The stone clearance has a significant relationship with the size of the stone and post-operative complications.
